How do you know when/what is saved in New Super Mario Bros. 2?

I just started the game, went to level 1-4 and then I shut off my 3DS to go bed... and when I started the game back up I was at level 1-1!

Is quick save the only way to keep your place? If so, I wish it permanently saved at least your highest level!!!!

Yes, quick save instead of just shutting it off.

After every mid-level fortress and end-level castle, I think. And once you beat the main game or final boss battle is when you get the "save anytime" option. Its been like that in most 2D Marios since the SNES era, but I recall saving in Super Mario Advance 4 (SMB 3) by going up the staircase in world 5 (if I really wanted to save).
