I'm afraid to check my Blind Fire. I literally shoot Ivana clips down empty hallways and start firing as I go around corners. Usually there is no one there, but you know, just in case.

Same here, rubber bullets and pre-firing around corners with mah ivana mean my accuracy is terrible. I think it's something like 10.83% ....it was around 15% before I got that gun lol

Improvised: Needs to be a direct impact kill from a grenade/flash/smoke. Easiest way to do this is on GE mode. Get behind someone, shoot them a few times with your pistol and chuck a grenade directly at them.

Reloaded: You just need to reload, I believe, 50x in a match. We did region on stuff one day, I just hid and shot single shots at the wall, and reloaded after each one.
