Do you have to run around in radiation or something?
No but if you kill Tenpenny in Tenpenny Tower, a new quest will open up to you when you leave the tower. Speak to the ghoul to complete the quest and you'll receive a ghoul mask!

statusAs of no, it is impossible to be a ghoul as far as I know. This may change once they release the constrution set, I'm sure someone will make a mod for it.
