Im pretty sure that u get the Fist of Ruin after defeating Kitana and the other 2 girls with her

When You Are In Out World Go Right Then Up Go Into The Blue Portal Then Defeat Kitana And Go To The Next Portal Then Kitana Jade And Millena Then Take The Coin See This Link If Your Still Stuck www.gamefaqs.com/console/ps2/file/925007/39743

After u defeat Kitana, Jade, and Mileena a coin will appear then grab it, and thats it.
