These are the requirements for the A rank...
The ranking system is based on points being awarded for how much time it took you, saves, and healing(first aid sprays are a huge deduction as opposed to herbs). I don't know the exact breakdown, but I can give you the MINIMUM requirements:
1.) 3 hours or less playtime
2.) 1 or less first aid sprays
3.) 3 or less saves
This is the easiest route to go...
If you break any of these requirements, then you must up your performance on the other two. I.E. You make 5 saves...so now you MUST finish the game in 2:30 or less and use NO first Aid sprays. Used 2 first aid sprays? 2:30 or less and NO saves. 4 hour playtime? NO saves and NO first aid sprays.
Now you notice that I seem to talk about first aid sprays more than herbs...this is because herbs are such a small part of the healing score that you would have to use an awful lot to see your rank go down...so use herbs before you use the sprays.
But, if you use the minimum requirements you should get it easily. Good luck...

statusIt's been awhile since I tried, but I will give it a shot...
1) You can use as many herbs as you want to, NO FIRST AID SPRAYS.
2) I THINK you are allowed to save only ONCE.
If you are playing the "B Scenario," my advice is to use it just before taking the single train cab car down to the umbrella labs since that boss on the way down can be quite cheap. You accomplish a lot of the game, without having to go back and do so much since the Umbrella lab is not very long. I digress...
3) I think your total time must be less than 2 hours and 30 minutes.
I suggest trying the LeonA / Claire B Scenario set up. I've done Leon A in under 2 hours. Claire B can met that time pretty easily. Good luck!
