What are the requirements to make him appear?
You must be playing on at least Hard difficulty(anything higher will do) and you must reach the Final Stage in 9 minutes or less. You must do this with <b>no</b> continues. As far as I know, Target Smash does not affect the time.

This is harder than in Melee. You must complete all Classic Mode (except the hand battle) in under nine minutes in a difficulty of Hard or higher.
I am not sure, though, how continuing affects the timer or if Target smash Stages are taken into the time limit or not.
You need to complete Classic mode under 9 minutes on hard or higher with no continues. When you battle the hands, they both appear at the same time.
Reach the final stage in 9min or less when playing on hard difficulty or higher.
In Classic Mode, choose the Hard Difficulty (stocks do not matter), and beat it under 8 minutes. You will face Crazy Hand, who is alongside Master Hand. Trust me, it is a super-hard battle. Each hand has around 300 HP, so be ready.
Reach Master hand in 9min or less in hard mode with no continues. The character you absolutley own with otherwise your gonna die
