In the QUEST "The Urn of Sacred Ashes" once you get near the end of the quest you will encounter a group of people from the Cult of Andraste. Dialogue will appear and you must persuade the man to let you join his cult. He will then give you blood to drop in the urn once you have reached it. Drop the blood in the Urn, defeat the guardian, then return to the cultist. You must then agree to whatever he asks of you and then he will give you the Reaver specialization.

There is also a Dwarf merchant in Denerim who you can purchase a manual off of to learn the speciality but don't forget you can't choose your specialty until level 7 then second one at level 14
What punisherbv said, keep in mind that if you don't want to use the blood on the urn in your playthrough and still want to unlock reaver without another playthrough you can save before using the blood play untill reaver is unlocked and then continue from the saved point on your planned route.
You also have too fight Leliana if she is in your party she disapproves of the blood in the urn so ull have too fight her also ... learned the hard way :D

Basicaly if you Put the blood in to urn you get reaver but cant get champion from are eamon and vice versa.
Also Wynne and Leliana will turn on you and attack you if you do the blood in the urn.
