What are all the different 'adventure-enders'?

As an immortal, you can't die normally. However, the manual states that there are some occasions where you can get a game over. What are all these occasions?

Irritating the Lady of Pain twice is the main way....if you annoy her once, she Mazes you; the second time, she ejects you from Sigil forever and the game ends. Among the ways to offend the Lady are:

*In the Alley of Dangerous Angles, you will find the Cathedral of a Dead God (really just another ramshackle building), and within is a man named Aola, the last worshipper of the god Aoskar. Aola can help you deal with an unwanted item, and he also offers to make you a disciple of Aoskar. If you become a disciple, the Lady will appear immediately after you leave the "Cathedral", and punish you for believing in another god.

*Go on a killing spree through the Hive; not just thugs, but random innocent passerby (try to avoid killing any "name" NPCs you might need later). I don't know exactly how much of a body count is necessary, but sooner or later, the Lady will appear to deal with you.

*I can't remember where, but somewhere in the Hive, you can buy a rag doll of the Lady of Pain. Once you buy it, examine it in your inventory, and choose to worship it when the text box offers you the choice. After doing this several times (upwards of a dozen), you'll be told that you feel an "icy chill"--when this happens, put the doll away. Apparently the Lady is a goddess that can't be bothered with namby-pamby crap like being worshipped, because she shows up and gives you the business.

You can also sit on the Silent King's throne. Since you can't experience True Death, you will be stuck there for all eternity and the game ends.

But, hey, at least the Dead Nations have a king, right?

There are a lot of ways in the final area (all of it, not just where you fight the boss), but most involove spoilers - they're fairly obvious, though.

One of the other ways is to try to threaten Lothar in the Lower Ward.
