Do you have to pay an extra fee to start a multiplayer server.

It depends on how many players you'd like on your server. With a decent computer you should be able to run a smaller server for just you and your friends at no charge, but if you're wanting a 24/7 permanent server to add to the community then yes, you'll probably have to purchase hosting services from a company.

No, you don't.
If you have your own machine, then no, but keep in mind you'll most likely want to have some sort of donations account setup in order to bring in some sort of income.
Bandwidth can start to cost quite a bit when you're running a server with 10+ people on it. Also, you may want to look into colocation of your machine if you're wanting a decent player count.
