No. The best you can get is rare. If you don't like the specialists or want a rare version, you can leave the hospital and check back until it is what you want, but you have to take all items at once, you can't choose only the rare ones.

Yes it is possible because i got a legendary Devil's ring from the hospital
I got a legendary item once. Also, the item's at the hospital are less valuable compared to the lifespan of your psp buttons, so go hunt elsewhere.
You can get any rarity from the hospital, it just takes entering and exiting enough time to get the item at the rarity yo want it.(it can take forever some times)
According to the official guide ( my bro gave it to me) the only rarities available from the hospital are common or rare. Legendary is not available.
