Before you go searching like a bafoon, let me say that these guys are right but for got ONE IMPORTANT DETAIL!!!! YOU MUST BEAT THE OPTIONAL BOSS IN SAID PLACE FOR HIM TO APPEAR, IF SAID PLACE HAS NO OPTIANAL BOSS THEN HE WILL APPEAR BY DEFAULT!!!!

Death only shows up from 2nd playthrough on, first playthough will have Traps instead of Death. Will show up about 5% of time as a dark presence in a random normal chest and will be asked if really want to open it. Doesn't show in the gold chests requiring keys. Magatsu Mandala, however, increases encounter rate to about 15% of time with best chances on floor just before Adachi. I've heard that if reverse Lovers all normal chests contain Death and he won't show once you've maxed out all the Social Links. Having Death Arcana couldn't hurt but I encountered Death while climbing the Steamhouse to rescue Kanji so beating Optional Bosses probably doesn't make much of a difference but again, couldn't hurt to do first.
