How do I achieve "You're Solid Gold!"?

I know you have to get 40 gold medals in a row, but does that mean 40 NEW gold medals, 40 gold medals without restarting or returning to the menu in the middle of something, both, or neither?

Answer

The gold medals do not have to be new. Nothing really helps to clarify what resets the counter but I would assume quitting in the middle of a stage or restarting the game does it.
