I got ALL gold medals and yet I didnt get the Achievement? (Stay alive from beginning to End)

I did each scenario perfectly, with all gold metals, and yet I still didnt get the achievement. What am I doing wrong?

Answer

You have to play the entire episode (not the individual scenarios) from beginning to end without dying.
