If I change the difficulty setting as soon as I get control of Sheperd at the start of the game will I still get the Achievement?
(ex Hardcore-->Insane at the beginning and then beat the game.)

There's no reason why you shouldn't unlock the achievement for beating the game on Insane if you change the game difficulty soon enough. I think the latest you can change the difficulty and still get the achievement, is before touching the beacon at the end of Eden Prime, but don't quote me 100% on it.
