People in the game keep telling me I look sick or tired. Do I really need to sleep?

They tell you you look sick because you are sick. Chug a cure disease.

look in your magic menu, added effect. Maybe you have something
I had Ataxia at one point and found out because an NPC told me LOL

They also do that if you make your Character REALLY pale.

If you download hacked games to not pay for them. You're a thief. Period.

If you mean the shopkeep of the potions place in Whiterun telling you you're sick, she just says that by default. If you mean general NPCs, check your active effects for any negatives to see if you're diseased and use a cure disease potion or a temple altar to cure yourself.

On my woodelf I used to get told that ALL THE TIME. One of the old lady's in Whiterun kept telling me that every time i passed her... no diseases on me and with the rested state on me. I finally just killed her to make her shut up.

On my Breton... never heard that from any NPC.

Just drink a Potion of Cure Disease, or visit a Shrine in any town. There's a Shrine of Talos in the upper square of Whiterun, and also the Temple of Kynareth very close by.

Pretty much any alchemist will tell you that you look sick, even if you're not. They are selling alchemical potions after all.
