I had to send my system in because of the green screen of death. I got my replacement yesterday, and I've had controller disconnects ever since. Doesn't matter what game I'm playing, the controller randomly disconnects and reconnects. I doubt it's the controller because this was never an issue with my Xbone that died.
If Platinum was HAND drawn, she'd be on paper, fool. This is a video game. They just made her on screen, no "drawing" involved -Delano7 on BlazBlue

Try re-syncing your controller with the replacement system.

I tried the power cycle where you hold power for 5 seconds, and I tried re-syncing the controller. Neither worked.

Could try the full shut off and unplug the console completely for at least 15 seconds, that seems to work for quite a bit of issues. Otherwise you may have to contact MS support, see what they say.

You could update the controller, probably won't help but worth a shot.
