The one I have is a little bent and doesn't fit in the case.

did it come out of the box bent, or did you do something to it.....

Who's the mobo or case manufacturer that the I/O shield came with? For an item as minimal as that I bet they would just mail one out to you. Then again most of us here aren't customer service representatives, so your best bet would be to call them up.
