Those enemies like Cliff Racers, or Cazadors that annoy you to no end.

Wall Witches in Deadly Premonition. OMG I cannot believe how many of them I had to fight and that the game forced me to learn how to be good at fighting them. Nah, can't just stumble through those fights, there's so many encounters you have to become an effective and efficient Wall Witch killer.

Tornadoes in ToeJam & Earl.

Chargers in L4D2.

Banshees in ME3.

River Krust in Dishonored.

Gaps in Skate.

The Ice Titan in LoS comes to mind since I just played that part yesterday. I couldn't figure out how to prevent it from knocking me off of its head when it tried to grab me.

If the other guys are referring to the Medusa Heads from to old GBA Castlevania games I may have to agree with that as well. Though its been ages since I've played one of them.
