In many games, I would play on Easy early in the days but now I do Medium/Normal. In Mortal Kombat games, I always do Very Easy because I suck at fighting games. In Guitar Hero/Rock Band games, I always play on Expert for Guitar and Bass but I may play Drums on Hard, Expert, or Expert+ and Vocals on any difficulty depending how hard the vocals are (I'm not good at singing).

It may be considered cheesy, but I almost always set it to the easiest setting first. I usually play games to be entertained, not challenged, but if I like a game enough, I might try to raise the difficulty and see how I do.

First on hard, switch to medium for those ever-present impossible QTEs.

Usually start on medium and will select hard if it a series I am familiar with but if the game proves to easy within like the first hour on normal I'll start again on a harder setting.

1st playthrough always normal
2nd playthrough easy to mess around and find potential new tricks and stuff
3rd onwards hard so i feel I've fully completed the game

I used to start on Normal back in the days when games were actually challenging.
However, these days I find myself starting on hard all the time without problems. These days, games are piss-easy after all.

If I like the gameplay, hard.

If I find the gameplay lacking and just want to experience the game for other reasons (story etc) medium/normal......or even possibly hard still......just depends on the game.

First play through is always on normal then after that I go through easy to get missed trophies than after that if I feel like it/have the time I will go through on hard.
