Only for games I truly adore. Getting 100% completion for every game would take too long. And most of the time I'm satisfied with a game once I beat the main story.

I try to get as close as I can on my first playthrough without checking a guide, or FAQs or anything. Just through normal exploration and trial-and-error.

If I miss things, and have put some good effort into finding them, I'll just miss out on them in the end.

Sometimes, on two conditions
1: I really like the game
2: 100% completion does not involve anything sadistically hard.

If I like the game enough though I'll go back and 100% it later with help to make sure I get everything.
