This game marks the first appearance of Carmilla, a recurring boss in the series. She is named after the main character of Sheridan Le Fanu's Carmilla, a vampire novel published in 1871. Her in-game appearance as a giant mask is a reference as to how the novel's character constantly wore a mask to keep her undead nature a secret.

Dracula's grave reads "1431-76." This is a reference to Vlad Tepes (Vlad the Impaler), a major inspiration for Bram Stoker's original Dracula character. Vlad Tepes was born in 1431 and died in 1476.
